I hereby certify that the information given by me on this employment application form and in the hiring process is true, accurate, correct and complete to the best of my knowledge and belief. I understand that if any of the information provided is discovered to be incorrect or false, it will be sufficient cause for my discharge should I become employed.
I hereby authorize and request any present or former employer, school, police department, division of motor vehicles, or other persons or agencies having personal knowledge about me to furnish you with any and all information in their possession regarding me, in connection with an applications for employment. I hereby release said companies or persons from all liability for issuing this information.
I understand that, if hired, I will be an employee at will and my employment can be terminated with or without cause or notice, at any time, at the option of either you or myself.
I understand that, if hired, I may obtain information of a proprietary or confidential nature relating to the present or future business or clients (“Confidential Information”), of Greater New York Hospital Association, Subsidiaries & Affiliates (“GNYHA”), the value of which will be destroyed or seriously diminished by improper use or disclosure. I understand that, if I am hired by GNYHA, I will use this Confidential Information solely for GNYHA’s benefit, subject to certain conditions and restrictions. I also may be subject to any additional restrictions regarding Confidential Information set forth in my hiring letter or other GNYHA policies.
I understand that a drug/alcohol screen, if appropriate, and background check, may be required following a conditional offer of employment.
