Summary/Objective
The behavioral health care manager is a core member of the collaborative care team, including the patient’s medical provider and psychiatric consultant, as well as the larger primary care team or medical team. The behavioral health care manager is responsible for supporting and coordinating the mental and physical health care of patients on an assigned patient caseload with the patient’s medical provider and, when appropriate, other mental health providers.

Facilitate referrals and care transitions to specialty behavioral health, substance use treatment, social services, or community resources as clinically indicated, and develop relapse prevention and self-management plans for patients achieving treatment goals or transitioning from the caseload.
Required Education and Experience
Master’s degree or higher in Social Work, Counseling, or Psychology from an accredited institution.
Minimum 1–3 years of clinical experience providing behavioral health assessment and treatment.
Required License/Certifications
Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or Licensed Psychological Associate (as permitted by Texas law)
