Job Overview: 

Coordinate the care of patients from preadmission to post discharge while meeting length of stay and occupancy rate targets.

Accountabilities:

  • Assess clients for appropriate admission to service and liaise with physicians and team.
  • Manage wait-list, occupancy rates and patient flow for the service considering alternate levels of care.
  • Liaise with referring agencies and provide ongoing support/education during the pre-admission phase using Resource Matching and referral (RM&R).
  • Co-ordinate the care of clients from pre-admission to post-discharge, regularly assessing needs such as psycho-social support for the patient and family as well as funding support while meeting length of stay and occupancy rate targets.
  • Ensure timely and appropriate clinical documentation in the electronic patient record (EPR).
  • Develop strong relationships community providers to facilitate partnerships between patients, family, inpatient clinical team and referring agencies (e.g. Toronto Public  Health).
  • Refer and coordinate consultant / discipline involvement as needed .
  • Support collection of clinical utilization data in collaboration with Service Manager.
  • Provide resource clinical leadership for the interprofessional team.
  • Provide leadership in program evaluation, program development and research as appropriate.
  • Participate in internal and external committees.
  • Partner with Service Manager to ensure clinical service goals are addressed.
  • Ensure that clinical care issues / concerns are addressed in a timely and effective manner.
  • Ensure that care is patient-focused with an emphasis on culturally appropriate care.
  • Perform other duties as assigned.
  • Provide coverage/support to Care Coordinators as needed.

Qualifications:

  • Bachelor of Science in Nursing preferred
  • Registration with appropriate regulatory college or eligibility for same
  • Member of relevant professional association/network preferred
  • Registration with appropriate regulatory college or eligibility for same
  • Must be able to achieve 80% in the Functional Independent Measure (FIM) certification exam within the probationary period and on recertification exams if applying to work on any of the following rehabilitation units:  3EA (Musculoskeletal Rehabilitation), 3EB (Amputee Rehabilitation), 3EC (Neurological Rehabilitation) and 2WF (Respiratory Rehabilitation)
  • Minimum of 4 years clinical professional background in a healthcare environment
  • Demonstrated clinical assessment skills including the ability to assess clients from pre-admission to post discharge
  • Knowledge of external agencies (e.g. Toronto Public Health) and resources (e.g. alternate discharge locations)
  • Demonstrated ability to partner and work effectively with all levels of staff, the interprofessional team, physicians, patients, families and external agencies
  • Demonstrated ability to deal with complex mental health and behavioral issues
  • Ability to maintain effectiveness in varying environments and with different tasks, responsibilities and people
  • Demonstrated knowledge of and ability to provide patient centred care. Demonstrated organizational, problem solving, interpersonal, coaching, negotiation and facilitation skills.
  • Strong organizational and prioritization skills to work independently in an unstructured, demanding environment
  • Demonstrated reliable work habits
  • Ability to work a flexible schedule
  • Strong Windows based computer skills e.g. Microsoft Office Suite
  • Fluency in a language other than English an asset preferred
  • Experience in a hospital/rehabilitation setting preferred
  • Evidence of satisfactory attendance record
  • An employee’s disciplinary record will be reviewed as part of the assessment of qualifications
  • Ability to work in a manner that exemplifies the Centre’s core values and helps patients get their life back
  • Should be aware of conditions and measures that may affect the safety of patients, residents, students, employees, volunteers, physicians and visitors
  • Knowledge of orthopedics/surgical experience preferred
  • Knowledge of community agencies (i.e., Home and Community Care, Public Health)
