As a dedicated Social Worker, you'll play a pivotal role in guiding and providing professional social work services to residents, patients and families towards improving quality of life, given the changes in their social, emotional, psychological and physical situation. Responsibilities: 

  • Provide comprehensive social work services, including assessment, counseling, and advocacy
  • Develop, plan, implement, and evaluate social work services as needed
  • Facilitate education and support groups for patients and families, occasionally during evenings
  • Collaborate with Care Coordinators to ensure smooth discharge and transition processes
  • Participate in pre-admission and admission assessments
  • Foster relationships with community providers for enhanced patient support
  • Provide resource clinical leadership within the inter-professional team
  • Contribute to program evaluation, development, and research
  • Partner with Service Managers to achieve clinical service goals
  • Address psycho-social issues promptly and effectively
  • Ensure patient-focused care with cultural sensitivity and inclusion

Key Qualifications: 

  • Master’s or Baccalaureate degree in Social Work 
  • Membership in Ontario College of Social Workers and Social Service Workers and a relevant professional association/network preferred.
  • Fluency in a language other than English an asset preferred
  • 1-2 years clinical experience, preferably in a hospital or rehabilitation setting
  • Knowledge of Chronic Care & Rehabilitation
  • Proven leadership abilities in resource coordination, community liaison, education, advocacy, and training
  • Strong assessment skills and knowledge of community resources
  • Experience in discharge planning and transitions implementation
  • Excellent interpersonal, organizational, and communication skills
  • Ability to work effectively with diverse teams and stakeholders
  • Strong problem-solving and coaching skills
  • Dedication to patient-centered care and adherence to core values
  • Ability to maintain effectiveness in changing environments and tasks
