Job Overview:

To assist clients to carry out various tasks needed to accomplish their learning goals, in collaboration with educators and under supervision of the Service Manager.

Responsibilities: 

  • Work in collaboration with the client, Life Skills Educator, Health Educator and Service Manager to assist the client to accomplish their learning goals and engage in community participation.
  • Provide support for clients to learn about grocery shopping (make lists, accompany to grocery store, shop on-line etc).
  • Provide support for clients to access community resources (assist with phone calls, on-line searches, accompany in the community when necessary).
  • Assist in the teaching of clients in the independent living philosophy and how to direct their own care.
  • Assist Life Skills and/or Health Educators with individual and group client workshops/special events as needed.
  • Establish good rapport and respectful relationships with clients while modeling client-focused service.
  • Document as required (charts, communication books, incident reports, work orders etc).
  • Function as an effective team member (participate in informal and formal meetings with clients, co-workers, families and multidisciplinary team working together to support clients to achieve their goals).
  • Perform other duties as assigned or delegated.
  • Maintain privacy and confidentiality of clients’ information according to standards of WPHC.
  • Be aware of conditions and measures that may affect the safety of clients, staff and visitors including:
    • Compliance with the safety standards of the hospital, applicable legislation and procedures as pertinent to Gage Transition to Independent Living at 100 Merton Street.
    • Identification and prevention of safety issues and problems.
    • Taking corrective action, where possible.
    • Attendance at safety meetings/training/updates in accordance with the Centre’s standards.
  • Ability to work in a manner that exemplifies the Centre’s core values and helps patients get their life back.
  • Evidence of good performance history and attendance record.

Key Qualifications: 

  • Personal Support Worker Certificate required
  • Demonstrated understanding of the philosophical basis of Independent Living
  • Demonstrated understanding and sensitivity to the client learning process
  • Strong interpersonal, organizational and communication skills (both written and verbal)
  • Demonstrated ability to maintain effectiveness in varying situations and with different tasks
  • Demonstrated ability to work effectively with clients and the trans-disciplinary team
  • Evidence of satisfactory attendance, punctuality and reliability
  • 3-5 years of experience working with people with disabilities in supportive housing settings preferred
