Job Overview:
The Clinical Pharmacist is responsible for providing comprehensive pharmaceutical care to all patients within the designated unit.
Accountabilities:
- Active participation in patient care rounds, offering pharmaceutical care services such as medication reconciliation upon admission and discharge, therapeutic drug monitoring and more.
- Cultivating and demonstrating clinical expertise tailored to specific patient populations.
- Providing valuable drug information and insights to physicians, nurses and other healthcare professionals.
- Meticulous verification of all medication orders, ensuring accuracy and safety.
- Serving as a preceptor for pharmacy students, nurturing their growth and development.
- Delivering patient education to promote optimal drug utilization and health outcomes.
- Coordination and involvement in quarterly medication reviews on designated units to enhance medication management.
- Initiating, instructing and monitoring patients in self – medication programs to foster self- care.
- Actively participating in hospital committees as necessary, contributing to the institutions initiatives and policies.
- Offering Saturday coverage and sharing on-call responsibilities in rotation with other West Park pharmacists, ensuring pharmacy services are available when needed.
- Maintaining a vigilant awareness of conditions and measures that impact the safety of patients, residents, students, employees, volunteers, physicians and visitors including:
- Compliance with the safety standards of the hospital and applicable legislation
- Identification and prevention of safety issues and problems
- Taking corrective action, where possible
- Attendance at safety meetings/training/updates in accordance with the Centre’s standards.
- Ability to work in a manner that exemplifies the Centre’s core values and helps patients get their life back.
Key Qualifications:
- Bachelor’s Degree in Pharmacy
- Doctor of Pharmacy (PharmD) preferred
- Registered as a Part A Pharmacist with the Ontario College of Pharmacists
- Designation certification an asset, such as geriatric pharmacist, anticoagulation pharmacist, antibiotic stewardship, diabetic educator, etc. preferred
- 2 years’ work experience in a healthcare setting
- Excellent computer skills
- Hospital pharmacy residency certificate preferred
