Job Overview:

The Clinical Pharmacist is responsible for providing comprehensive pharmaceutical care to all patients within the designated unit.

Accountabilities:  

  • Active participation in patient care rounds, offering pharmaceutical care services such as medication reconciliation upon admission and discharge, therapeutic drug monitoring and more.
  • Cultivating and demonstrating clinical expertise tailored to specific patient populations.
  • Providing valuable drug information and insights to physicians, nurses and other healthcare professionals.
  • Meticulous verification of all medication orders, ensuring accuracy and safety.
  • Serving as a preceptor for pharmacy students, nurturing their growth and development.
  • Delivering patient education to promote optimal drug utilization and health outcomes.
  • Coordination and involvement in quarterly medication reviews on designated units to enhance medication management.
  • Initiating, instructing and monitoring patients in self – medication programs to foster self- care.
  • Actively participating in hospital committees as necessary, contributing to the institutions initiatives and policies.
  • Offering Saturday coverage and sharing on-call responsibilities in rotation with other West Park pharmacists, ensuring pharmacy services are available when needed.
  • Maintaining a vigilant awareness of conditions and measures that impact the safety of patients, residents, students, employees, volunteers, physicians and visitors including:
    • Compliance with the safety standards of the hospital and applicable legislation
    • Identification and prevention of safety issues and problems
    • Taking corrective action, where possible
  • Attendance at safety meetings/training/updates in accordance with the Centre’s standards.
  • Ability to work in a manner that exemplifies the Centre’s core values and helps patients get their life back.

 Key Qualifications: 

  • Bachelor’s Degree in Pharmacy
  • Doctor of Pharmacy (PharmD) preferred
  • Registered as a Part A Pharmacist with the Ontario College of Pharmacists
  • Designation certification an asset, such as geriatric pharmacist, anticoagulation pharmacist, antibiotic stewardship, diabetic educator, etc. preferred
  • 2 years’ work experience in a healthcare setting
  • Excellent computer skills
  • Hospital pharmacy residency certificate preferred
