The successful candidate will plan, manage, implement and monitor population health projects. You will strategise, engage and work together with diverse groups of stakeholders and with all levels of staff to overcome challenges and deliver results in a dynamic and fast-paced environment.
 

Job Responsibilities

• Manage project activities, plan and monitor progress to ensure that the project on track.
• Actively monitor and analyse clinics’ population health performance, develop and deploy effective strategies and execution plans across clinics
• Work closely with various stakeholders to ensure clinic KPIs and project deliverables are met; prioritise timeline, resources and resolve issues as they arise. 
• Track and consolidate population health implementation outcomes for report submissions and dashboard maintenance.
• Lead Workgroup to establish Operations framework, workflows and evaluate the program needs and effectiveness to support the Population health initiatives. 
• Develop care coordinators competency, including career progression planning.
• Proactively fine tune workflows with clinic stakeholders in initiative implementation to achieve operational efficiency.
• Assist to prepare justification and budget submissions during budgeting cycle.
• Disseminate new work policies, instructions and ensure clinics’ compliance to work instructions.
• Understand & anticipate the potential changes, impact & opportunities in application systems arising from the concepts & decisions from Workgroups.

Job Requirements

• Degree in any business related discipline 
• At least 5 years of relevant experience in business management. Experience in the healthcare industry, especially in areas of business administration will be a plus.
• Excellent leadership, negotiation, project and people management skills. 
• Proven track record of improving operational workflows leading to higher efficiency 
• Strong in data analytics skills.
• Proficient in the use of Microsoft Office, preferably in the areas of data visualization and data analytics tools like Tableau and Oracle BI.
