Responsibilities:
- Manage ESRD patients in a dialysis facility or at home (HD/PD) by engaging the patient, conducting interviews, gathering data, conducting case assessments (using the BPSS, risk & needs assessments, and other appropriate assessment tools), conceptualising the case, planning the case, and developing intervention strategies for the best patient outcomes.
- Make visits to dialysis centres, homes, or hospitals to provide supportive counselling.
- Provision of supervision and/or training to Social Workers to enhance quality standards of ethical and best practice clinical standards.
- Work collaboratively with healthcare professionals on a professional level within the organisation.
- Responsible for Service Management administration, ensuring accuracy and alignment of means-testing results across systems.
- To assist in the administering and approval of welfare and social assistances.
- Responsible for department and organisation’s KPI accomplishments.
- To collaborate extensively on program-related issues with internal departments and outside partners.
- To produce reports from various systems to support internal and external audit reviews.
- To streamline work processes and suggest process modifications that are required to uphold accountability and effectiveness.
- To prepare statistics and reports for both sporadic and ongoing reporting/ bulk approval.
Requirements:
- BA in majoring Social Work/Graduate diploma in Social Work
- Registered Social Worker (RSW) with the Singapore Association of Social Workers
- Experience in working in a social service agency, with at least 3 years in direct social work practice.
- Experience in administration and accounting related work.
- Strong knowledge and experience in Excel – master formulas, v lookup, pivot tables, data analysis etc
- Independent, self-motivated and a team player

- Positive mind set with a can-do attitude.
- Good direct case management abilities Able to work under pressure with minimum supervision.
- Excellent communication and interpersonal skills
