Responsibilities:

• Perform nutrition screening with reliable screening tools to evaluate patient risks, level of criticality and to direct services while adhering to the Standards of Practice (SOP) in Nutrition Care.
• Conduct nutritional assessments with suitable methods which may include:

o   identify and record medical, anthropometry, biochemical, social and environmental data necessary to plan nutritional management

o   perform diet interviews to obtain a comprehensive diet history, 

o   conduct Subjective Global Assessment (SGA),

o   participate in the interdisciplinary comprehensive assessments.

• Integrate foundational dietetics knowledge with critical appraisal of assessment data to diagnose nutrition problem(s) which can be resolved or improved through treatment or nutrition intervention.
• Develop nutrition plans and implement interventions (e.g. diet counselling) based on knowledge of patient’s current health and nutrition problem(s).
• Provide client follow-up and modify nutrition care plan as necessary.
• Involved in continuous planning, execution and improvement of nutrition projects and programs (e.g. outreach program, group education program, etc)
• Review literatures and/or utilize appropriate educational resources (e.g. workshop, conferences) to update knowledge and develop skill.
• Perform other duties as assigned.

Requirements

• Bachelor or advance degree with major studies in food and nutrition or dietetics and completed clinical dietetics internship (at least 24 weeks) in approved institution; A member of Singapore Nutrition and Dietetic Association and obtained yearly certificate of Accredited Dietitian in Singapore (advisable)
• 1 year experience in clinical nutrition (preferred)
• Proficient in Microsoft Applications.
